A third worker has died from an explosion at a Foxconn plant in China that makes Apple products, the contract manufacturer said on Sunday.

The company said it is still investigating the blast, but said initial findings suggest “that the accident was caused by an explosion of combustible dust in a duct.”

Foxconn said it has halted production at that plant and similar plants pending the investigation. The facility is believed to make Apple’s iPad.

Statement from Foxconn Technology Group

Update on the Tragic Accident at Foxconn Technology Group’s Hongfujin Precision Electronics (Chengdu) Co. Facility

Foxconn can confirm that, sadly, a third employee has died from injuries from the May 20 explosion at one of the polishing workshops at our company’s Hongfujin Precision Electronics (Chengdu) Co. Ltd. facility in Chengdu.

Fifteen other employees were injured in that accident and six of those employees have been treated and released from the hospital. Foxconn is working with medical professionals and the local government to ensure that all of the injured employees receive the highest quality medical treatment. Our condolences go out to the families of the deceased employees and they are being given the full support of the company. Our thoughts are also with the injured employees and their families who are also being given the full support of our company at this very difficult time.

The cause of this tragic accident is still being investigated by a joint investigation task force led by government officials and law enforcement authorities, but that task force has communicated initial findings that the accident was caused by an explosion of combustible dust in a duct. Foxconn is cooperating fully with all relevant government bodies to carry out a full investigation into the specific root cause of the accident and the company is taking all necessary actions to ensure the safety of employees at this and all other production facilities.

All operations at the affected workshop remain suspended and production at all other workshops that carry out similar processing functions have also been halted pending the results of the investigation into the cause of the Chengdu accident. All other production operations in our facilities in China continue operating normally.

Foxconn Technology Group will provide updates on this tragic accident as information becomes available.

Amazon won’t come out and say exactly what happened to its sales-ranking system over the past few days. But it is sorry, and it would like the Web and its customers to know that it wasn’t singling out books aimed at gays and lesbians. Here’s the apology:

This is an embarrassing and ham-fisted cataloging error for a company that prides itself on offering complete selection.

It has been misreported that the issue was limited to Gay & Lesbian themed titles–in fact, it impacted 57,310 books in a number of broad categories such as Health, Mind & Body, Reproductive & Sexual Medicine, and Erotica. This problem impacted books not just in the United States but globally. It affected not just sales rank but also had the effect of removing the books from Amazon’s main product search.

Many books have now been fixed and we’re in the process of fixing the remainder as quickly as possible, and we intend to implement new measures to make this kind of accident less likely to occur in the future.

That isn’t going to mollify Amazon’s most vocal critics in the blog- and Twitter-spheres or the ones who ascribe the screw-up to either homophobia or nefarious hackers. But it’s almost certainly going to be the last word from Amazon (AMZN), which is about as tight-lipped as a publicly-held retailer can be.

Wall Street analysts have to beg Amazon to part with even basic financial details. If you think Jeff Bezos and company are going to get into the workings, and failings, of their online catalog, I admire your optimism.

Suntech (STP) tonight said that seven of its employees were hurt on Nov. 28 in an accident in its module production facilities in Wuxi, China. The solar company said the injured have been hospitalized. The company did not give any details on the nature of the incident.

However, earlier this afternoon–hours before the announcement–research firm Wedge Partners reported in a research note that one employee was killed (Update: the company denies there were any fatalities) and seven injured in an explosion on a cell-diffusion line, a processing step in PV cell manufacturing that uses high temperatures and combustible gases. The Wedge Partners note said that the seven injured workers suffered severe burns.

You’d think that with all the text messaging bans and new hands-free cellphone usage laws being adopted these days a motorist would know better than to text while driving, especially if that motorist also happens to be a commuter train engineer.

Tragically, that’s not the case.

The Metrolink engineer responsible for the worst U.S. train crash in the last decade sent and received 45 text messages while helming his train the day of the deadly collision in California last month–including one just moments before running a red signal and crashing head-on into a freight train. According to the National Transportation Safety Board, the last message the engineer received was at 4:21:03, more than a minute before the accident. The last one he sent was at 4:22:01, just 22 seconds before the trains, traveling at some 40 miles an hour, collided.

Rail experts were aghast at the news. “For me, it just gives me heart palpitations thinking about it,” said Tim Smith, California chairman of the Brotherhood of Locomotive Engineers and Trainmen. “The last thing you want to be doing is something that takes your eyes off the road.”
